Job summary An exciting opportunity has arisen to join the cardiac rehabilitation team o based within the community setting. It is a part time post working over various sites making driving an essential part of the role.The post holder will provide cardiac rehabilitation in conjunction with the multidisciplinary team and maintain a high standard of clinical leadership, taking charge of own patient caseloads as required.You will support nurse led services and the development of the cardiac rehabilitation service. Main duties of the job Assess, plan & evaluate individualised care with patients following on from a diagnosis of coronary heart disease. To facilitate Heart manual, Revascularization Manual and Angina plan. Td families and carers. To provide a high qualityo contribute to the development of local protocols and pathways based on evidence such as NICE and BACR. Participate in delivering pathways of care in patients homes and community venues through phase 2, 3 & 4 of cardiac rehabilitation. Deliver primary preventative and secondary preventative strategies and advice to patients anservice to support people who are ready to change their exercise lifestyle. We provide integrated hospital and community based services to round 400,000 people living in East Durham, Hartlepool, Stockton on Tees and surrounding areas including Sedgefield, Easington and Peterlee. Person Specification Qualifications Essential Registered nurse level 1 Teaching qualification Desirable Post registration cardiac course Independant prescribing Clinical skills Experience Essential Post registration experience within a cardiac setting Health promotion Knowledge of National Service Framework for CHD. Desirable Cardiac rehabilitation Skills Essential Computer literacy Evidence of personal development Desirable Use of S1
