An exciting opportunity has arisen for 2 full time equivalent Care Co-ordinators to join the Reablement team, which is part of the Support at Home Service, on a fixed term basis.
Successful applicants will be supported by a Team Manager and will work in collaboration with colleagues from other disciplines within the team which will comprise Occupational Therapists, Social Workers, rehab and technology staff.
The team will be responsible for the assessment of people who are being discharged from hospital. Assessment will take a strengths-based approach and post holders will be encouraged to think creatively about how we support service users during their period of assessment. This will include ensuring service users meet eligibility criteria.
Successful applicants must have the ability to work as part of a team, respond to the demands of a busy service, be able to follow procedures, have excellent communication and good report writing skills. They must have the ability to travel throughout West Lothian and hold a recognised qualification in social care.
The skills and knowledge applicants must have are: -
SVQ 3 or an HNC in Social Care or relevant equivalent work experience.
It would be desirable for candidates to have relevant assessment and care management experience in an adults or older peoples social work practice team.

A sound knowledge of what is available within communities and how this can be accessed to provide support and meet support needs would be desirable
This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
