Care Coordinator | Birmingham | Full Time | £25,000

This role has many aspects with the principal duties being to ensure the efficient running of the care service in a manner that demonstrates high standards by exceeding the needs of the customer.

Excellent organisation and communication skills
Excellent telephone manners and the ability to work under pressure
You must always observe strict and complete confidentiality with staff and service users' personal information
Must be computer literate and able to input data on the internal rostering system
A genuine concern for the welfare of others

What will your role look like?
To organise the care workers' rotas whilst considering the changing needs of service users
To clearly set out the expectations for each visit and direct the care workers accordingly
To promote dignity, respect and choice for service users ensuring that the care workers always work within the ethos of the company
To be accountable to the Registered Manager ensuring that they are kept informed of any concerns or changes to the health and well-being of individual service users
Ensure that care and support are provided for service users in a timely way, upholding and improving standards of care delivery
To act as a role model and be an example to the care workers at all times
To have a good understanding of service users' needs and be able to respond effectively and responsively to changing needs
Monitoring the ECM to ensure care workers are logging in and out
Monitoring of MAR charts on Care4IT and contacting care workers if there are any missed logs
To carry out Risk Assessments and Care Assessments and to formulate Person Centred Care Plans
To promote good communication among the care workers
To carry out Supervision sessions and Appraisals on all care staff
To identify training needs and organise Training and Development plans for all care workers
Carry out care worker forums
Send out staff and service user surveys gaining feedback on performance
Attend service users' reviews and update care plans accordingly
To be involved in carrying out Disciplinary hearings with care workers when misconduct occurs
To adhere to all Safeguarding policies and procedures
To abide by all CQC regulations
