Care Coordinator
Description
Gian Healthcare is looking for a Care Coordinator who will ensure the delivery of a high standard of Care across their Supported Living and Domiciliary care services in the Manchester and Stockport Area.
The Role
Accountable to: Deputy Manager, Registered Manager and Director
Responsible for:Support Staff
Key responsibilities:
Answering incoming telephone enquiries regarding client care.
Processing new client referrals.
Being a Liaison between service users ( and relatives), professional partners, and other stakeholders
Carrying out Risk assessments and assisting with care plans
Ensuring relevant matrixes are up to date at all times
Arranging new Service User assessments and care plan reviews when required
Scheduling Weekly care visits for our community-based staff
Identifying capacity, bidding on and scheduling new care packages
Contributing to our On-call / out of hours service
Assisting senior management in the effective and efficient operation of the service.
Assisting senior management in creating strategies that reflect the departmental staffing needs
Providing strong leadership and support to staff
Monitoring health and safety in the workplace and in the field
Actively participating in the implementation, quality monitoring and improvement of systems.
Assisting with the investigation of complaints and workplace incidents, and writing of reports.
Providing direct support to identified higher risk clients who require support in specialist services.
The Person
The ideal candidate should have:
An excellent understanding of the social care sector especially homecare services.
Knowledge of the principles of a high-quality person-centered care, and anti-discriminatory care practice
An excellent understanding of the regulatory requirements and the law relating to domiciliary care services
Knowledge of the health and safety and Risk management practices relating to homecare services
Knowledge of safeguarding procedures
Successful experience of leading and managing teams delivering health and social care services
Knowledge in all the CQC domains
Essential:
Must have right to live and work in the UK
Providing care: 1 year (Required)
Driving Licence (Required)
Ms Office Suite (Required)
Care coordinating: 1 year (Preferred but not essential)
