The Clinical Case Manager will be a driven, motivated UK registered nurse (RSCN or RGN), with strong clinical, management and communication skills to safely manage a caseload of children and young people with complex healthcare needs across our growing service.


The role requires a commitment to quality and high standards of care and service delivery.

 Working with children and young people with complex healthcare needs (and their families), the Clinical Case Manager will assess, plan, implement and evaluate evidence-based, family centred care and collaborate with professionals across agencies and sectors ensuring safe and effective service delivery.


The Clinical Case Manager will also provide essential training and support to staff working in packages to ensure safe care delivery through our in-house Education Pathway.

Childrens Complex Care are dedicated to staff development and CPD. In this role, all required training and development is paid for including DBS and annual NMC fees.


Role Responsibilities

  • Safe and effective management of a clinical case load in the community setting;
  • Complete assessments and bespoke care delivery plans.
  • Clinical Governance and Risk Management
  • Work as part of the Education Team in providing classroom based one to one/ bespoke training to community staff where required;
  • Educate, support and complete competency assessments for non-registered healthcare staff on clinical delegated tasks;
  • Carry out staff appraisals and supervisions for registered nurses and Healthcare Assistants;
  • Effective engagement and collaborative working with ICB's and external MDT;
  • To ensure nursing practice is in accordance with the NMC Codes of Conducts;
  • Clinically accountable and responsible to ensure all reasonable measures are in place to deliver safe effective evidence- based care to all children/ young people in collaboration with all colleagues including on call and Bookings Team, to the best of yours and the organisation's capabilities
