Job Description:

Early career pharmacists who are yet to achieve a postgraduate certificate (or equivalent) will be considered for this post on a band 6 to 7 development basis. Banding will be reflective of level of qualification/experience.

We are seeking a Clinical Pharmacist with an interest in mental health to be based at Langdon Hospital. Langdon Hospital, a purpose-built facility located within extensive countryside, cares for adult males with a serious mental illness. A stay can include admission and assessment of acutely unwell patients, the varied road of recovery and planning good quality continuity of care at discharge. An admission can last for months to years which gives a rare opportunity to build trust and give the time needed to become fully embedded within a multi-disciplinary team which seeks to improve both mental and physical health. The role allows for a high level of involvement from the Medicines Optimisation team with a high percentage of time being patient-facing especially with no dispensary working. Due to the various co-morbidities that our patients have, including a diverse age range, exposure and involvement within the management of physical health conditions is key presenting an exciting opportunity to continue professional development.

The site also employs a wide range of staff with different skills and specialities, allowing the Medicines Optimisation team to work in partnership within a MDT where the opinions and work of pharmacy is sought and valued.

Main duties of the job

The post holder will support patients and their families by providing advice about the actions and use of medication, optimising treatment, providing information and liaising with acute and other care providers to ensure seamless transfers of care. By attending daily MDT meetings to support and influence prescribing decisions whilst concurrently working with the patient, it creates a unique position to positively impact patient care. Often, due to the nature of the patient's presentation and history, it is necessary to work outside of standard published guidelines therefore a keen eye for detail is required to identify and assess primary research. Working within the Langdon Medicines Optimisation Team, currently consisting of a pharmacy technician, a clinical prioritisation technician and a senior clinical pharmacist, provides great immediate support with integration within the wider DPT Medicines Optimisation Team for regular training and development opportunities.

This role has no requirement for on-call, weekend or late night working at present, allowing for a good work life balance with excellent job satisfaction.

Job responsibilities

Job Purpose

The post holder will provide a specialist clinical pharmacy service to Mental Health and Learning Disability inpatients at Langdon Hospital (Secure Services). Where there are service needs, the post holder will also be expected to provide specialist clinical pharmacy services to other areas across the Trust.

In doing so they will:

• Implement the Trust Medicines Optimisation (MO) strategy so that agreed, robust systems and processes are in place to ensure safe, high quality and cost-effective MO services.
• Ensure that these systems meet the requirements of the people who use our services, their relatives and carer, the Trust and the regulatory bodies.
• Assist with the development, implementation, audit and evaluation of MO policies, standard operating procedures, clinical protocols, practice standards, safety briefings and prescribing guidelines.
• Provide expert advice on specialist pharmaceutical matters and manage risks associated with medication; including provision of information regarding medicines use, expenditure and place in treatment to promote cost effective, evidence based prescribing with support from the Senior Clinical Pharmacist and wider MO Team.
• Provide information and advice about medicines to patients and/or their carers to promote informed consent and ensure safe and effective use of medicine and minimise waste.
• Address and resolve interface issues with primary care and other organisations, to ensure accurate medicines reconciliation and smooth transfer between care environments.
• Contribute to incident reporting and reviews.

This post is open to early career pharmacists on a band 6 to 7 progression development basis. For the right candidate, the post can be for a Foundation Pharmacist (band 6) and therefore a training post suitable for entry level or newly qualified Pharmacists. There will be an expectation for the post holder to undertake personal development based on the HEE Foundation pharmacist curriculum, which includes completion of a postgraduate qualification in mental health pharmacy if this is not already held.

Qualifications

• Successful completion of pre-registration/trainee pharmacist program
• Current pharmacist registration with the General Pharmaceutical Council.
• Evidence of Continuing Professional Development.
• Member of the College of Mental Health Pharmacy
• Postgraduate diploma in clinical pharmacy / therapeutics.
• Postgraduate certificate/diploma or masters in psychiatric pharmacy practice

Experience

• Experience of working within a multidisciplinary healthcare team / environment.
• Experience of undertaking patient-facing consultations conveying complex information.
• Ability to supervise activities of pharmacy technicians and assistants.
• Managing change in a complex environment and evidence of having successfully implemented change.
• The ability to work under pressure, to deadlines where there are conflicting priorities.
• Previous experience of mental health conditions and medications used in their management.
• Experience of undertaking clinical audit.
• Experience of delivering training programmes.

Knowledge

• Demonstrates clinical knowledge and skills.
• Up to date knowledge of current NHS policies.
• Evidence of post registration pharmacy practice, knowledge and experience to demonstrate competency and autonomy in a broad range of clinical and pharmaceutical knowledge.
• Willingness to obtain further qualifications in mental health or pharmacy practice if not already held as identified through the post holders personal development plan.
• Knowledge of relevant health legislation and awareness of current issues in mental health and learning disability services.
