Community Nurse -Eastleigh/Winchester - Hampshire
Nurse Seekers are proud to be working alongside an exceptional community healthcare provider who services their community with a focus on compassionate, patient-centred care. They are now looking to recruit a Nurse (RGN) to join their dynamic team and contribute to the care of their service users.
Job Description: Manage a varied caseload, including patients with complex care needs. Deliver high-quality nursing care in the community, collaborating with patients, families, and healthcare professionals.
Key Responsibilities:
* Conduct health assessments and develop care plans.
* Provide direct nursing care and manage chronic diseases.
* Support patients with complex needs, including palliative and post-operative care.
* Educate patients and families on health maintenance.
* Coordinate with multidisciplinary teams.
* Maintain accurate patient records.
* Participate in community health initiatives.
Qualifications:
* Registered Nurse (RGN) with up to date PIN
* Minimum 2 years of nursing experience, preferably in community or with Complex Care
* Strong clinical, communication, and organisational skills.
* Ability to work independently and in a team.
* Valid driver's license and reliable transportation.
