Community Mental Health Practitioner
Opportunity for a Registered Mental Health Nurse to work a community team, Based in Northampton
The team sits within a community hospital with the role predominately working within a dynamic team carrying out new assessments and care coordinate for patients. As part of this role you will carry out assessments of mental health needs, care planning with psychosocial interventions and medication and you will be out in the community providing care coordination so a valid driving license with access to a vehicle is essential. It is also essential you have experience working within the community. The ideal candidate will be someone who has excellent assessment and communication skills, and who is able to work dynamically within a fast paced, challenging yet rewarding team.
Shift patterns for this role are Monday to Friday with the role being full time and the aim to start ASAP
The hourly rate for this role is: £25.00 Per Hour
