Job Title: Community Registered Nurse

What would you be doing at HomeLink?
- You'll be seeing patients in their home environment.
-You will be responsible for contributing to the holistic assessment, planning, delivery and evaluation of patient care.
- Provide personal and clinical care including IV therapy, cannulation and wound management.?
- Initiate health promotion activities, as appropriate, with individuals, patients and carers.
-Communicate effectively with patients, carers and all members of the multi-disciplinary team.
- Monitor and record patients' conditions by taking observations and escalate appropriately.
- Work autonomously in the community with your patients.
What would we expect from you?
- NMC registered with a minimum, of 3 years experience.
- Right to work in the UKwithout any form of sponsorship.
- You must have a drivers licence and access to a car, this is essential as its a mobile role.
