Home Care Direct are industry-leading specialists in personalisation. We help a wide variety of people to take control over their care at home using personal budgets, personal health budgets, direct payments or their own funding.
About the post:
This is an exciting opportunity to join a national company that specialises in supporting clients with complex care needs in their own home. We provide bespoke packages of care so our personal assistants are trained to a high level to support their specific clients needs, they do not travel to multiple homes as per a traditional domiciliary care provider.
This vacancy is for a Community Nurse Assessor, working in the Brighton and Hove area to work 4 to 5 days per week, supporting clients at home with complex health care needs. Our client base includes people living with learning disability, autism, mental health concerns and long-term physical disabilities.
About your duties:
The role of a Community Nurse Assessor is to provide ongoing visits for your client caseload, create support plans, complete risk assessments, personal assistant interviews and inductions, supervisions and annual client reviews. There is a requirement to also train our personal assistants to meet the clinical needs safely. There are MDT meetings with local professionals including the ICBs, Hospices, Allied Health teams, GP Practices, Social Work Teams, Safeguarding and Court of Protection, and Families.
You will oversee the clients care and train the staff to deliver safe care. Whilst managing your own caseload you will be making clinical decisions, holding meetings, raising safeguardings if required and working to ensure our clients are given the best opportunities to live well and achieve their own goals. Although you will be working autonomously, you will be supported by our other Brighton based nurse and a care facilitator that manages all non-clinical aspects of the package of care. You will be given full training and mentoring will be available to help you grow into the role
About you and our requirements:
You will need to be a competent and confident practitioner that has experience of working alone or in senior roles. We pride ourselves on being proactive and supportive, and work in partnership with all stakeholders.
You must have a minimum of 2 years community experience and have a registered NMC active PIN with registration as an RGN, RMN, LDN.

PBS Practitioner - To create a PBS plan to support an individual and their team.
Presentation Skills - To new clients Meet and Greet and/or to Commissioners
HR Skills - With HR instruction: Undertake PA Interviews/Disciplinaries/Dismissals
Occupational Competency includes a minimum of two Level Three skills

£44,850 - £48,750 per annum, based on 37.5 hours per week depending on experience and qualifications
