Job Description
Work as a Community Nurse Case Manager for a complex care provider, supporting clients to live independently at home and within their wider communities. The role is covering the Merseyside region.
The Nurse Case Manager role will manage a caseload of clients with complex needs in the community. The role is varied and entails;
Assessments
Care planning
Training & assessing workers competencies
Working with the wider MDT
Attending meetings and making referrals
Holding team meetings
We are looking for an RGN Qualified Nurse with excellent clinical skills including peg feeds, ventilators and tracheostomy care. You will ideally have experience in training healthcare assistants or nurses.
All sites are rated as Good with CQC and some with elements of outstanding!
To apply for this role, you must be a car driver as the role is community based.
This community role is to work full time mainly Monday to Friday 9am - 5pm with some flexibility required, and is home based with travel across Merseyside.
The salary is negotiable dependent on experience, circa £34,000 - £38,000 with the opportunity to earn up to £6000 performance bonus each year
