The Crisis and homebased treatment team has the primary aim ofensuring that a high-quality alternative to acute psychiatric admission existsfor those individuals in the locality who are experiencing a crisis related tosevere mental health problems. The service is gatekeeper to all Inpatient bedsto allow consideration of alternatives to Inpatient care. The service also aimsto provide an effective In Reach Service to facilitate early discharge fromhospital. A member of the Crisis/IHTTeam regularly attends the Inpatient 24 Hour Report out and provides clearcommunication links between the two services. A member of the Crisis/IHT Team alsoattends Discharge Planning Meetings as appropriate and works with the CareCoordinator to facilitate discharge. It is a 24-hour, 365 day per year serviceand is capable of a rapid response seeing all urgent referrals within 4 hours.
The team ismulti-disciplinary, working with people aged 18-65yearswithin the York and Selby location. It utilises the expertise of doctors, nurses, ooccupational therapists, psychologists, socialworkers, and support workers.The post is supported by a nurse consultant (who is a prescriber) and a staffgrade section 12 approved doctor. The post also accommodates a core or GP traineeand a Higher trainee post.
There is an expectation that the Consultant will attend teamdaily huddles which is where patients are discussed, Consultants will berequired to support the team on a day-to-day basis and review patients as andwhen necessary. The Consultant will also attend weekly Supercell meeting(attended by Team manager, Consultant, Advanced practitioner, Nurse consultant,Psychologist etc.) to discuss team and patient related issues. The leadershipteam works as a collective leadership so the post holder should have anappreciation of a horizontal hierarchy.
The post holder takes part in assessments, follow-up and joint working of patients to provideclinical leadership to themultidisciplinary team.
The post holder provides input to multidisciplinary discussions of assessments and management plans
The post holder works jointly with colleagues from other disciplines and all relevant agencies toprovide a comprehensive approach to patient care and treatment.
The post holder contributes to postgraduate teaching and supervision of junior doctors.
The appointee must be approved under Section 12(2) MHA 1983.
The appointee must be able to travel independently.
