The Wokingham Community Older Adults Mental Health Team is looking for an enthusiastic, caring and highly motivated Mental Health Practitioner (Mental Health Nurse, Social Worker, Occupational Therapist) to join our friendly and innovative Community Team The Wokingham Older Adult Mental Health Service is a multi-disciplinary team with specialist skills in caring for older people with mental health needs in the community. The post holder will take responsibility for the assessment and implementation of intensive care and treatment in the homes of people with functional mental health problems, as well as people with dementia. The successful candidate will have the relevant clinical experience and must display a passion and understanding for working with Older People, appropriate training and support will be provided as needed. You must be enthusiastic, caring and highly motivated to contribute towards maintaining standards of high-quality care to our patients. In our latest CQC inspection we achieved 'Outstanding' and therefore the successful candidate would be expected to work towards helping the service maintain these accolades of high standards Main duties of the job The successful candidate will be responsible for managing their own case load, monitoring the mental health needs of patients and working alongside their carers and assessing care needs under the Care Act. Knowledge of appropriate legislation such as the Care Act, Mental Capacity Act and Mental Health Act is key to this post. To monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team. To provide specialist consultation and advice on issues relating to the care of older people with mental health problems to other professionals within the wider care community such as Residential Homes, Sheltered Housing, Day Centres etc. To have knowledge of therapeutic interventions such as crisis intervention and problem solving About us Berkshire Healthcare NHS Foundation Trust is a specialist mental health and community health services trust. Person Specification Education Qualifications Training Essential Registered Mental Health Practitioner - Nurse / OT / Social Worker / Speech and Language Therapist Desirable Mentorship Experience Desirable Experience of working with older people with mental health needs. Primary nursing experience. Experience of working in the community. Knowledge, Skills & Abilities Essential Understanding of the effect of mental illness on patients and carers. Desirable The care of people with dementia. The care of people with functional illness Competences In Essential Good communication skills Ability to plan, implement and evaluate care plans.
