Team Leader - Primary Care Network (PCN) Service Ref: NWM/162
? What We Offer:
Full time 37.5 hours per week
?Location: South Norfolk
?*Due to the nature of the role a driver licence and access to a vehicle is essential for this role, mileage is paid at 45p per mile for cars after your home to work base mileage is deducted.
?Salary Offered: £24,874.50 rising to £26,334.53 following successful completion of probationary period.
?Annual Leave entitlement of 26 days rising to 29 days with service

The Service
?This is an exciting service based in GP surgeries located across Norfolk and Waveney. You will be responsible for overseeing the operational management of a team of five recovery workers based in one of five areas, working in strong partnership with local surgeries and Primary Care Networks (PCNs)to see a reduction in the number of people with mental health issues re-presenting to their GP. This will include support / signposting to access Hub-based and Wellbeing services to build increased community resilience for the service user and freeing up GP and secondary services consultation time for wider system benefit.
? Among Other Responsibilities You Will Be
To support the management of mental health issues in primary care settings (such as within your GP practice). This will be delivered through a team of Recovery Workers based in your area.

Supported by the Service Manager to ensure that there is effective engagement across the Primary Care Networks and Multi-disciplinary teams within your area. This will include managing the team to work alongside the Primary Care Network (PCN) staff wider team and Norfolk and Suffolk Foundation Trust (NSFT) staff support to see a reduction in the number of people with mental health issues representing to their GP. This will include support / signposting to access Hub-based and Wellbeing services to build increased community resilience for the service user and freeing up GP and secondary services consultation time for wider system benefit.
Ensure the promotion of non-medical solutions and mental wellbeing, its role in self-management, addressing health inequalities and the wider determinants of health and therapeutic interventions and how they can reduce pressure on statutory services, improve health access and outcomes and enable a holistic approach to care. Particularly, but not exclusively, with mental health services
Ensure the provision of a quality recovery focused approach to people who may present with complex needs for example anxiety or depression alongside other issues - inadequate housing, drug or alcohol misuse or debt problems etc.; supporting people to self-manage long term health conditions using a range of techniques and in a non-judgmental way respecting diversity and lifestyle choices.
