Job summary East Kennet PCN has a total population size of approximately38,000 patients consisting of 4 practices who provide a collaborative way ofworking across the PCN. The PCN provides excellent support to all teamsand works in collaboration with The Pharmacist Network team. You will besupported by the practice and the current pharmacist provision in-house as wellas Senior Pharmacist support from The Pharmacist Network. As apharmacist, you will be aligned to 1 or 2 practices where you are able to makea real impact within the practice and develop your experience and expertisetoo.We currently have an exciting opportunity available for highly motivated and focused Clinical Pharmacist to support with the smooth running of day-to-day activities remotely for GP practices across the UK.If you have experience working in General Practice and preferably your Independent Qualification, we would like to hear from you. This is a Onsite working role suitable for candidates looking for permanent work. Main duties of the job In recognition of the important role clinical pharmacists can play in general practice we are seeking a professional pharmacists who can support the GP practice with general day to day workstreams using clinical systems such as Emis and System 1, workload may include the below:Undertake medicine reconciliations of patient record Prescribing in line with the practice protocols Update patient records, support medicine reviews and synchronization for groups of patients , care homes Help patients to get the best from their medicines by switching the medications to agreed and approved protocols. Carry out prescribing audits and medicines safety alerts. Supporting with Medication ReviewsIf you are looking for a remote opportunity, have previous experience of working in a similar clinical pharmacist position and have a natural desire to provide an excellent service with outstanding communication skills. Job description Job responsibilities You will work as part of a multi-disciplinary team in a patient-facing role, working with colleagues to help resolve medication-related issues, helping to utilise skill mix, improve patient outcomes and ensure better access to healthcare.You will support practice staff with prescription and medication queries. Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.You will support with prescribing practices across the PCN, providing clinical leadership on medicines optimization and quality improvement.
