Job summary This role is being advertised to grow our paramedic team, working for the PCN, at Kennet & Avon Medical Partnership in Marlborough and Pewsey. There is a well-established duty team with paramedics, nurse practitioners and GP's, this role will work within this team and will be required to treatpatients on the same day medically urgent list. There will also be a requirementto carry out some home visits. Main duties of the job Paramedics work autonomouslywithin the practice using their enhanced clinical assessment and treatmentskills, to provide first point of contact for patients presenting withundifferentiated, undiagnosed problems relating to minor illness or injury,abdominal pains, chest pains and headaches. They are health professionals whohave the capability to make sound judgements in the absence of full informationand to manage varying degrees of risk when there are complex, competing orambiguous information or uncertainty.There will be a requirement to work a full day every Friday. About us This role will join a team of paramedics, nurse practitioner and duty GP's whoare responsible for treating same day, medically urgent patients. The role willbe based from either site and will also include some home visits. Job description Job responsibilities 1. assess and triage patients,including same day triage, and as appropriate provide definitive treatment ormake necessary referrals to other members of the PCN team2. advise patients on generalhealthcare and promote self-management where appropriate, including signpostingpatients to other community or voluntary services.3. be able to: a. perform specialist health checks andreviews b. perform and interpret ECGs; alongsideother results as appropriate c. perform investigatory procedures asrequired d. undertake the collection of pathologicalspecimens including intravenous blood samples, swabs e. perform investigatory procedures neededby patients and those requested by GPs 4. provide an alternativemodel to urgent and same day home visits for the PCN and undertake clinicalaudits5. communicate at all levelsacross PCNs and other organizations, ensuring effective, patient-centredservice6. communicate proactively andeffectively with all colleagues across the multi-disciplinary team, attendingand contributing to meetings as required7. collaborate with othermembers of the PCN including doctors, nurses and other AHPs, acceptingreferrals and referring to them for specialist care8. maintain accurate andcontemporaneous health records appropriate to the consultation, ensuringaccurate completion of all necessary documentation associated with patienthealth care and registration with the practice9. prescribe, issue and reviewmedications as appropriate following policy, patient group directives, NICE(national) and local clinical guidelines and local care pathways. This will bedone in accordance with evidence-based practice and national and practiceprotocols, and within scope of practice10. work with patients inorder to support compliance with and adherence to prescribed treatments11. provide information andadvice on prescribed or over-the-counter medication on medication regimens,side-effects and interactions12. assess, diagnose, plan,implement and evaluate treatment/interventions and care for patients presentingwith an undifferentiated diagnosis, and patients with complex needs13. clinically examine andassess patient needs from a physiological and psychological perspective, andplan clinical care accordingly14. prioritise health problemsand intervene appropriately to assist the patient in complex, urgent oremergency situations, including initiation of effective emergency care15. diagnose and manage bothacute and chronic conditions, integrating both drug- and non-drug-basedtreatment methods into a management plan16. support patients to adopthealth promotion strategies that promote healthy lifestyles, and applyprinciples of self-carePersonal attributes1. enhance own performancethrough continuous professional development, keep up to date with currentevidence-based practice, and impart own knowledge and skills to PCN colleaguesto meet the needs of the service2. be verified against theparamedic primary care roadmap3. recognise and work withinown competence and professional code of conduct as regulated by the HCPC andCollege of Paramedics4. assess effectiveness ofcare delivery through self and peer review, benchmarking and formal evaluation5. participate in research andutilise the audit cycle as a means of evaluating the quality of the work ofself and the team, implementing improvements where requiredAdministrative requirements1. produce accurate,contemporaneous and complete records of patient consultation, consistent withlegislation, policies and procedures2. be aware of data protection(GDPR) and confidentiality issues particularly within a PCN3. use technology andappropriate software as an aid to management in planning, implementation andmonitoring of care, presenting and communicating information4. review and process datausing accurate read codes in order to ensure easy and accurate informationretrieval for monitoring and audit processes.SupervisionThe postholder will haveaccess to appropriate clinical supervision and an appropriate named individualin the PCN to provide general advice and support on a day to day basis. Person Specification Qualifications Essential BSc in a training programme approved by the College of Paramedics Health & Care Professions Council (HCPC) registration Able to operate at an advanced level of clinical practice Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11 Desirable Pre-reg MSc in a training programme approved by the College of Paramedics Non-medical prescribing qualification Full UK driving license Evidence of verification against the paramedic primary care roadmap Experience Essential Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans. Working knowledge of Microsoft and GP practice and prescribing data monitoring systems. Ability to write comprehensive clinical notes, implement and evaluate care plans. Desirable Working towards advanced clinical practitioner status. Mentorship or supervisory skills training. Ability to undertake nails surgery assessments, procedure and post- operative care. Be aware of data protection (GDPR) and confidentiality issues particularly within a PCN. Cognitive behavioural and motivational interviewing approaches / skills.
