Job summary The post holder will be an Advanced Practitioner, or a PrescribingNurse or Paramedic, who will work clinically providing and developing the HomeVisiting Service, whilst also providing clinical support to the PCN FrailtyTeam. The post holder will work closelywith the PCN Leadership Team and wider Health and Care Partners.The Home Visiting Service supports patients registered withinCarlisle PCN which covers approx. 38,000 patients registered with three generalpractices as below:1. Eden MedicalGroup2. CarlisleCentral Practice3. Warwick SquareGroup Practice The Home Visiting Service provides a combination of acute on theday, urgent and routine visits. Inaddition, it is expected that the post holder will support our Frailty Team whoprovide a combination of visits covering acute, clinical review, chronicdisease management, proactive caseload management and care planning, to ourFrailty population. This includes weeklycare home ward rounds to our PCN aligned care homes; nursing and residential.The PCN Team also works with our health and care partnersincluding the NCIC trust, Adult Social Care and voluntary sector organisations. The PCN team consists of ACPs, Nurses, Frailty CareCo-ordinators, Social Prescribers, Pharmacy Technicians and Long-termConditions Co-ordinators. Seniorclinical advice will be provided by the Lead ACP and all 3 practices have Main duties of the job The post holder will provide an innovative, integratedservice to patients living within the Carlisle PCN boundary, who have beenidentified as requiring further support. The post holder will work closely with thePCN Frailty Team and GP Practices to provide patient centred care, primarilyproviding a Home Visiting Service. Therole will involve triaging and assessing patients withundifferentiated and undiagnosed problems, initiating treatment(including prescribing) and investigations, providing advice and management,and liaising where appropriate with other health and care professionals. The post holder will provide appropriatepatient care within their scope of practice and in accordance with practice protocolsand policies. The post holder will actively engage patientsand carers in the management of their health, promoting health and wellbeing.The post holder will be responsible forrecognising, negotiating and implementing change processes to enhance thedelivery of care to the Carlisle PCN population.The post holder will contribute todevelopment and delivery of a high quality innovative service to patientswithin the community through the implementation of evidence based clinicalpractice, supported by continual professional development, as part of the widermulti-disciplinary team. Job description Job responsibilities Professional Role To promote evidence-based practice through the use of the latestresearch-based guidelines and the development of practice-based research. To maintain their professional registration and keep up to datewith mandatory training. To record accurate consultation data in patients records inaccordance with the latest regulatory guidance an other pertinent standards. Pro-actively promote the role of the ACP, including externally tokey stakeholders and agencies.Clinical Role To undertake visits to patients as requested by the practices. To assess patients within their role and make professionallyautonomous decisions. Provide a first point of contact alongside the Frailty team forNursing and Residential Care Homes. Provide regular ward rounds and telephone calls/visits to CareHomes and anticipatory care visits to Housebound as per the post-holders caseload. Instigate necessary diagnostic tests or investigations whereappropriate and interpret findings/reports and act upon them appropriately. Reviewing patients where needed and makinglong term care plans. As an independent prescriber: to prescribe safe, effective and appropriatemedication as defined by current legislative framework. To work with the medicines management team to provide and/orensure polypharmacy medication reviews and de-prescribe where appropriate. To provide safe, evidence-based, cost-effective, individualisedpatient care. To provide advanced care and respect planning and End of life carewhere appropriate. To promote health and well-being through the use of diseaseprevention and screening, health promotion and health education as part of anMDT. To refer patients directly to other services/agencies asappropriate. To work with the MDT to ensure practice and PCN contractualrequirements are delivered and met for patients within caseload and in supportof our wider population. To work with the MDT to ensure our care is compliant, safe, ofhigh quality and follows regulatory guidelines at practice and PCN level. To participate in identification of community health needs aroundour population health data and the identification if health inequalities anddevelop patient, family and/or community centred strategies to address them aspart of our MDT. To help develop and set up new patient services and participate ininitiatives to improve existing patient services at PCN and locality level.Training & Educational Currently enhancing professional development and to maintain apersonal development plan. Continually keeps up to date with current evidence based practice Is involved in lifelong learning Participates in in-house training provided and completion of allmandatory training at clinical and practice levels. Keep up to date with pertinent health- related policy and workwith the practice team to consider the impact and strategies forimplementation. Training requirements will be monitored by yearly appraisal andwill be in accordance with practice requirements. Personal development will beencouraged and supported by the PCN. It is individuals responsibility toremain up to date with recent developments. Maintain continued education by attendance at courses and studydays as deemed useful or necessary for professional development ensuring PREPrequirements are met.
