P ost holder will be employed by Saxonbrook Medical Centre .

Reports to: GP Partners clinically, Practice Manager Administratively

Proposed salary: £34,000 - £38,000 per annum dependent on experience (pro rata)

Hours of work: 37.5 hours per week

Base:
Saxonbrook Medical Centre, Maidenbower Square, Maidenbower, Crawley, West Sussex, RH10 7QH. We also operate from other sites within Crawley.

Saxonbrook Medical is a 20,000 patient population practice serving our community over two sites. The team is comprised of Doctors, Physician Associates, Paramedics, Pharmacists, Nurses and Healthcare Assistants all supported by a strong administrative team. We work together as a team to provide the best care possible to our community. We are a teaching Practice who welcome a number of trainees into the Practice including Doctors, Student Physician Associates and Student Nurses and also support the progress and development of our employees.

We work within a local Primary Care Network, working collaboratively with Social Prescribers, Pharmacists and First Contact Practitioners.

We offer the NHS Pension Scheme. Clinical staff receive 25 days annual leave + bank holidays pro rata. There is also a provision for study leave.

Job summary

Saxonbrook Medical Centre have an exciting opportunity to expand their Primary Care Workforce by employing a Practice Nurse. Our aim is to provide exemplary patient care and find innovative solutions in general practice to deliver the best care we can for our patients.

The post holder will work as part of the primary healthcare team providing Personal Medical Services to the patients of Practice.

Primary duties and areas of responsibility

  • Assist in and perform routine tasks related to patient care as directed by Nurse Practitioner and GPs
  • Knowledge of chronic disease management – Asthma, COPD, hypertension
  • Diabetes (not essential but knowledge helpful) – training can be given
  • Advise patients on general healthcare, including minor illnesses and ailments with referral to General Practitioners as necessary (training can be given)
  • Cytology – smear taking, swabs, basic sexual health & contraception
  • ECGs
  • Asthma/Peak Flow Readings. Spirometry helpful but training can be given
  • Routine immunisations/Childhood immunisations/B12s, flu and pneumonia, shingles immunisations, basic travel etc
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician
  • Requesting pathology tests, for example urine culture, swabs
  • Following agreed clinical protocols with referral to GPs as appropriate

Other tasks will include:

  • Following all infection control procedures and ensuring everything is in place, regular checks etc. Leading on Infection control for the practice
  • Keeping all emergency drug trolley checks and supplies up to date, and carrying out regular weekly checks on all equipment and supplies
  • Maintaining and cleaning equipment used by the nurses and GPs along with the Health Care Assistant – e.g. ear syringe machine, couches, minor op equipment, ECG machine, BP monitors etc
  • Maintaining GP and Nurses rooms, stocking and rotating items as required, ensuring cleaning schedules are carried out, filled in and completed
  • Chaperoning and assisting patients who are being examined by another clinician
  • Maintain general tidiness and cleanliness of nurses and treatment rooms
  • Maintain stock of waiting room leaflets and update/rotating notice boards with health promotion material
  • Ordering of Health Promotion Literature
  • Ordering of stock from the Hospital
  • Ordering of vaccinations to maintain stock levels
  • Participation in administrative systems in the Practice
  • At all times there will be a need to maintain accurate records
  • A duty to advise GPs of potential problems or errors within the range of assigned tasks
  • Attend and participate in any Practice meetings when required
  • On occasion there may be a requirement to undertake home visits
  • Any other delegated duties appropriate to the post

Person Specification

Professional Registration

  • Registered General Nurse

Qualifications

  • Recognised qualification to diploma level or above
  • Qualifications/Certificates in Asthma, hypertension, COPD management
  • A demonstrable commitment to professional development & Primary Care
  • Current PGDs, anaphylaxis, resuscitation and life support, training in chronic disease management, cytology and immunisations
  • Diabetes and Nurse triage

Skills, Knowledge and Experience

  • Experience of Primary Care
  • Experience of a Primary Care clinical system
  • Experience with COPD/Asthma/Hypertension
  • Experience of Microsoft Office software
  • Experience of dealing with the public/patients
  • Infection control
  • Competent in basic nursing duties required for the post & infection control
  • Ability to work without direct supervision and determine own workload priorities
  • Ability to work as part of an integrated multi-skilled team
