Job purpose summary

Working closely in a team with other similarly qualified colleagues (RMNs and RGNs), to ensure that appropriate outcomes are achieved when dealing with patients by promoting the values, culture and ethos of the organisation, using skills and experience to provide support, signposting and practical advice with due regard for medical protocols.

To assess each patient individually and produce and carry out the most appropriate plan for their wellbeing.

Key tasks:

1)

  • To contact patients within specified time following receipt of any new case, depending on advice and information given by individual schemes.
  • The scope of RedArc should be described to patients fully and agreements reached with the patient as to the best course of action.
  • To support and advise by telephone, all patients to the best of your ability ensuring advice, support and information given to patients is pertaining to individual needs and requests, making full use of your skills in general nursing when appropriate.

  • Ensure that all communication (whether written or verbal) with patients and third parties such as scheme sponsors and other health professionals is confidential, professional and well documented at all times in the ‘PIP’ system (computerised record keeping system).
  • Record all written/email documentation to patients on ‘PIP’.
  • Ensure ‘follow up’ dates to patients are recorded and met.

3)

  • Where an intervention is agreed (e.g. Counselling, Speech & Language Therapy, Physiotherapy, Complementary Therapy etc.) ensure procedure follows ‘Service Standards’ and ‘Service Level Agreements’.
  • Discharge patients appropriately and as required on ‘PIP’ system.

4)

  • To liaise with the Senior Nurse / Head of Nursing regarding any concerns over patients. Any complaints received about any aspect of the RedArc service should be reported immediately as per complaints policy.

Other tasks

  • To attend and participate in staff meetings as required.
  • To be involved in regular clinical supervision, 1-1s and annual appraisals as requested.
  • To be a good team member by providing and receiving peer support day to day
  • To be aware of own needs for training for professional development and discuss any requests with Senior Nurse.
  • To make yourself familiar with RedArc’s polices and procedure and adhere to them at all times.
  • To represent the organisation in a positive way, when dealing with patients, colleagues and scheme sponsors.
  • Maintain confidentiality at all times.
  • Comply with regular audits to ensure what we are doing is standardised, consistent and gives measurable information that we are meeting clinical governance standards
  • To attend and participate in case reviews and complete any learning outcomes identified.

Due to the changing nature of the business, the job holder may from time to time be required to undertake other activities of a similar nature that fall within their capabilities.
