Anglia Case Management is registered with the Care Quality Commission as a Domiciliary Care and Rehabilitation provider and is regularly inspected by the CQC; we have been judged to be Outstanding since 2017!We have a wonderful opportunity for an experienced Rehabilitation Case Manager in Chelmsford and surrounding areas to join our friendly team who work with adults and children who have ongoing needs, to support them to improve their quality of life, and maximise their potential. Anglia Case Management are committed to creating individual solutions for people who have been affected by injury or illness. We provide the expertise and experience to help clients maximise their potential and adapt to their circumstances. For this role you will need to be a driver with ability to travel and hold a UK professional health qualification which requires you to be HCPC, NMC or Social Worker Registered. It would also be an advantage if you are an Advanced Member or working towards advanced membership status of the British Association of Brain Injury Case Managers (BABICM).The range of services offered by Anglia Case Management has expanded from its original Case Management base, to include Expert Witness reports, Rehabilitation, Manual Handling assessments and training, and Mental Capacity Act assessments. We work with clients with a wide range of disabilities including brain injuries, spinal injuries, cerebral palsy, amputation, complex orthopaedic injuries, stroke, dementia, and PTSD.
