Job summary This is an exciting opportunity to join out Transfer of Care Team. You will be required to carry out a wide range of duties to support the Transfer of Care Hub in the delivery of a high quality care and effective discharge planning. The focus for the Senior Case Manager is to manage and direct, under supervision of a Lead Case Manager, the gathering of accurate and detailed information to support discharge plans over a seven day period. While clinical skills are not required for this role you will be patient facing in gathering the information and liaising with families / next of kin in order to complete robust assessments. This is a fixed term/secondment position until the 30th March 2025. Internal applicants currently employed by ESNEFT will be offered on a secondment basis only. Main duties of the job To maintain up-to-date knowledge of all discharge pathways including the DOH Hospital Discharge policy and primarily the Discharge to assess model. To be an advocate for the patient and their familyTo manage and prioritise your own case load while supporting fellow Case Managers with theirs to ensure patients discharges are appropriate to needs and completed in a timely mannerTo work as part of The TOCH to support discharge planning and system flow within a seven-day service-optimising patient's accurate and robust patient records are maintained and electronic records are kept live and up to date. Reflecting and promoting the Criteria to Reside in everyday an initial understanding of The Mental Capacity Act and its implications for work on your own initiative to ensure discharge planning for patients commences as soon as possible after establish and maintain working relationships and effective communication across a range of internal and external stakeholders including: Transfer of Care Hub, Ward staff, Matrons, site operation, Pharmacy Service, Therapy Services, Doctors, Nursing and Residential Care Homes, Voluntary support services and any other relevant teams or services. Person Specification Experience Essential Working in a busy working environment as part of a large team Ability to prioritise an unpredictable work load and work to time scales in a fast pace environment without losing attention to detail Desirable Previous experience of a co-ordinator role Previous experience in interagency and customer liaison Previous experience in a healthcare environment Qualifications Essential GCSE grade 5/C or equivalent in English and Maths NVQ level 3 in administration/secretarial or equivalent Desirable Any relevant vocational qualifications ( NVQ ) Evidence of Previous further study Knowledge Essential Understanding of the DOH Hospital discharge Policy - including discharge pathways / criteria to reside Knowledge of the Mental Capacity Act and implications for discharge Desirable Understanding of medical terminology
