Leaders in Care are looking for a Senior Community Nurse to join a nationally recognised provider in Kettering. The role comes with a fantastic benefits package, including a salary of up to £36k.

They deliver their patients lifesaving treatments, in the comfort of their own homes. As a Clinical Transfer Coordinator, you will be responsible for assessing patients suitability to complete their inpatients pathways in their homes.
In your first few weeks in this Clinical Transfer Coordinator role, you can expect to:
Work as part of our early discharge support team who work closely with a local hospital
Take charge of smooth discharges of patients
Complete clinical assessments of patients in the ward and determine the patients suitability for transfer home
Compile care plans and transition care to the multi-disciplinary teams.
To apply, you must be a Registered Nurse with a full UK driving license. The following are also required:
Experience administering IV antibiotics
Experience with complex wound care, including TNP VAC therapy.
